MyTERA does have an Occupational Health Survey that is used to determine how the MyTERA team can help you with occupational health services or products to help keep you safe from occupational health hazards. You will have the option to fill out a survey where the following information will be collected:
- Are you exposed to occupational health hazards in your workplace or at home? Option to select listed health hazards.
- Have you or your company done risk assessments for the selected health hazards and your respective tasks that are performed to determine what controls (i.e., engineering, administrative, or personal protective equipment) are needed to reduce the risk of exposure?
- Have you or your company done the required sampling or monitoring (e.g., air sampling for chemicals or sound surveys for noise) for the selected health hazards?
- Have you or your company done the required health surveillance (e.g., audiograms, respiratory function tests, blood tests, biological monitoring, or allergy tests) for the selected health hazards?
- Check what major industry you work in.
- How many employees your company has.
This is general information that the MyTERA team can use to determine how to help you. This information is not considered to be Protected Health Information (PHI) as there are no specifics to the concentration or amount of exposure that you may be exposed to, nor does it ask for the health status of you as an individual. It is used to determine how to help you, or your company be in compliance with applicable health standards and regulations.
The following Personal Identifying Information (PII) will be asked for so that the MyTERA team knows how to contact you or your company:
